Quality Coordinator

JOB DESCRIPTION:

POSITION SUMMARY:
In accordance with our 5-Star Quality Measure, our goal is to continuously maximize our efforts to ensure successful completion of the measure. The Quality Coordinator will be a vital component in successfully meeting our 5 Star measure. The candidate will need to have a high level of analytical and communication skills, attention to detail, ability to multi-task and meet deadlines.

FULL TIME: Monday-Friday 8am-5pm

DUTIES AND RESPONSIBILITIES:
• Reconcile patient visits and claim completion
• Request medical records
• Update 5 Star patient gap list
• Immediately inform supervisor of gap list discrepancies
• Re-direct patients in the network
• Assist & ensure completion of client requests
  ○ Reports
  ○ Unable to reach patients
  ○ Visit and claim verification
  ○ Patient outreach
  ○ Gathering of specific gap list details
• Cross-train in other quality areas
  ○ Vision Screening
  ○ Patient Experience

EDUCATION & EXPERIENCE:
• Associates degree preferred
• Must have intermediate to advanced MS Excel skills and be comfortable with Excel formulas and tools
• Experience in a health care environment is a plus.

• Bilingual (Fluent in both English and Spanish) preferred
